Request for PROPOSAL (RFP) - Medicaid Alternative Care (MAC) and Tailored Support for Older Adults (TSOA) Case Management

 

Snohomish County Long Term Care and Aging is soliciting responses from vendors for the contract of Medicaid Alternative Care (MAC) and Tailored Support for Older Adults (TSOA) Case Management. The County invites interested agencies to submit proposals to provide MAC and TSOA case management services to vulnerable persons, age 55 and over, and their unpaid caregivers who receive pre-Medicaid funded Long Term Services and Supports in their homes through the MAC or TSOA programs.

 

A description of the qualifications required, timeline for Request for Proposal (RFP) process and response instructions can be found online (https://snohomishcountywa.gov/1038/Request-for-Proposals-RFP) starting Tuesday, May 14, 2019. Any questions regarding this RFP should be directed via email to Aime Fink, Long Term Care and Aging Supervisor, at aime.fink@snoco.org.

 

A letter of intent to apply for RFP must be submitted by 4:00 pm local time on Friday, June 7, 2019. Organizations not submitting a letter of intent by due date and time will forfeit their right to apply for the RFP.
