Dementia Adult Day Facilities Request for Qualifications

The Aging and Disability Services Division of the City of Seattle Human Services Department (HSD) is accepting applications from State-approved adult day facilities interested in providing dementia adult day services for adults age 60 and older with Alzheimer’s disease or related dementias. This Request for Qualifications (RFQ) is open to any legally constituted entities that meet the minimum eligibility requirements. Services in adult day facilities may include help with personal care, health monitoring, skilled nursing, social services and activities, education, therapeutic activities, a nutritious meal and snacks, coordination of transportation, first aid, and emergency care. Approximately $337,585 is available through this RFQ. HSD intends to fund up to four (4) proposals. Initial awards will be made for the period of April 2, 2018 to March 31, 2019. While it is the City’s intention to renew agreements resulting from this RFQ on an annual basis through 2021 program year, future funding will be contingent upon performance and funding availability. The City of Seattle Human Services Department seeks to contract with State-approved adult day facilities to help ensure that older adults with memory loss maintain independence while providing support for their caregivers.
