2. Vendor Fact Sheet -Form is to be completed and emailed back with proposal.
3. Section 3 Business Certification Form / Resident Employment Plan – Form must be completed indicating whether or not your firm is a Section 3 firm and any plans to hire Section 3 Eligible residents and emailed back with proposal.
4. Suspension and Debarment Compliance Certificate for Consultant / Subs – Form must be completed and emailed back with proposal.
5. For Profit Subgrantee and Contractor Certification and Assurances – Form must be completed and emailed back with proposal.
