RFP : Child Wellness SurveyPROJECT DESCRIPTION: The purpose is to solicit qualified contractors to collect data for a new population-based survey, the Child Wellness Survey (CWS) in the State of Washington (WA). The primary purpose of this survey is to assess the health and well-being of children and families, with particular emphasis on demographic variation and inequity. A parent/caregiver familiar with the child’s health will answer questions about a pre-selected child; the children will be between six (6) months and 11 years old and reside in WA.

BIDDER ELIGIBILITY: The Washington State Department of Health is seeking qualified contractor to follow specific protocols for data collection, handling, and dataset preparation. Other activities include generating written summary reports, providing technical assistance and guidance on best practices for questionnaire development and data collection, and participating in quality improvement activities.

PERIOD OF PERFORMANCE:  The period of performance of any contract(s) resulting from this RFP shall be [November 1, 2023, through June 30, 2025. Amendments extending the period of performance, if any, shall be at the sole discretion of DOH.

AVAILABILITY OF DOCUMENTS: The RFP along with any amendments and bid results will be available only through Washington’s Electronic Business Solution (WEBS).  All interested Bidders must be registered with WEBS under the following commodity codes in order to receive automatic notifications: 918-12, 918-76, 948-76, 952-77, 956-75, 961-53, 961-60.

PROPOSAL DUE DATE: Thursday, August 24, 2023, by 2:00 p.m. Pacific Time.
