The Area Agency on Aging & Disabilities of Southwest Washington (AAADSW) is designated by the State of Washington as the Area Agency on Aging to serve Clark, Cowlitz, Klickitat, Skamania and Wahkiakum Counties. AAADSW’s mission is to promote independence, choice, wellbeing and dignity for persons aged 60 and over, and adults with disabilities and their families through a comprehensive, coordinated system of home and community-based services. Service providers under contract with AAADSW are overseen by the agency to meet the needs of the region’s elderly population (see AAADSW’s web site at www.HelpingElders.org).
Request for Price and/or Rate Quotes: This notice represents a formal request for price and/or rate quotes from qualified dental providers to provide oral health services to people age 60 or more in Clark County, Washington. Services may also be provided to residents of Klickitat and Skamania Counties who travel to receive care in Clark County.
Qualified Service Providers/Eligibility: Organizations responding to this announcement must provide comprehensive dental services to eligible seniors in the specified service area (Clark County, Washington), and must also comply with all applicable local, state, and federal laws, regulations, licensing, policies and contract provisions. Additional requirements will be addressed if the respondent’s information results in a contract.
Organizations that have been debarred, suspended, proposed for debarment, declared ineligible or excluded from participating in a contract by any federal or Washington State agency will not be considered. This opportunity is open to comprehensive dental providers only; subcontracting is not allowed.
Mandatory requirement to apply: Dental providers interested in responding to this notice must request a mandatory “Request for Price and/or Rate Quotes” form, which seeks information from qualified dental providers. This form can obtained by sending a request to AAADSWcontracts@dshs.wa.gov, and specify in the subject heading “Senior Oral Health Program Price and/or Rate Form,” or by submitting a brief written request for the “Senior Oral Health Program Price and/or Rate Form” to the following address:
